River Hills Home Health is an Agency Model PCA (Personal Care Assistant) service. Whether you’re receiving care or supporting a loved one, our trained PCA care providers work alongside you to promote independence and quality of life. Our caregivers deliver hands-on, in-home support, while our agency coordinates services, staffing, and ongoing care so you don’t have to manage it alone.

CFSS Services
We provide Community First Services, continuing the PCA services many families know and rely on. Our Rochester and Twin Cities home care workers help with:
- Activities of Daily Living (ADLs)
- Personal hygiene and grooming support
- Mobility assistance and transfers
- Medication reminders
- Meal preparation and light household support
- Ongoing supervision to support safety and independent living.
Licensed Home Care
