245D Waivered Services - Enabling Your Independence
At River Hills Home Health, we understand that maintaining independence is important for individuals with disabilities and their families. That's why we offer 245D Waivered Services, which provide assistance, supervision, and care to ensure the health and safety of individuals enrolled in the program.
245D Waivered Services are provided under basic and intensive service categories and may include 24-hour emergency assistance, adult companion services, homemaker services, night supervision, personal support, respite care, and other services tailored to each individual's unique needs.
Our team of experienced staff works with each individual and their families to develop a personalized care plan that meets their unique needs and preferences.

Community First Services and Supports (CFSS) - Providing More Choice and Flexibility
The Minnesota Department of Human Services is currently transitioning from PCA to CFSS (Community First Services and Supports). CFSS is a new program that will provide more flexibility and choices for individuals and their families to access home and community-based services.
CFSS is similar to Personal Care Assistance (PCA), but it expands the options for how services are provided and who can provide them. It offers additional support for writing plans, more self-direction options, and the ability to purchase goods to aid the individual maintain their independence. This program provides individuals with the opportunity to tailor their care plan to their unique needs and preferences. We are excited to provide CFSS services as soon as the transition is complete.

- Self-Directed: CFSS allows individuals to have more control over their care, including the ability to hire and manage their own caregivers.

Personalized Care for Independent Living with PCA
we understand that individuals with disabilities may require additional support to perform daily tasks. That's why we offer Personal Care Assistance (PCA) services to help with the activities of daily living (ADLs), such as dressing, grooming, eating, bathing, transferring, and toileting.
PCA is a type of in-home care that helps individuals with disabilities to maintain their independence by assisting with daily living tasks. PCA services are available to both children and adults. Our PCA staff are trained to provide personalized care and support, and can also assist with light housekeeping, laundry, and meal preparation services for adults.
