WHAT WE OFFER

245D Waivered Services

Minnesota’s 245D waivered service program is designed to support people with disabilities and older adults who live at home rather than in a nursing facility or institution.
River Hills Home Health is licensed to provide Basic Support Services and Intensive Support Services under the 245D waivered service program. The main difference between Basic Support Services and Intensive Support Services is the caregiving goal.

About Our Service Model

River Hills Home Health offers Community First Supports and Services (CFSS) under the Agency Model. CFSS has replaced the former Personal Care Assistance (PCA) program. As a recipient of care, the main difference for you is greater flexibility in how you receive support and services. 

Under the Agency Model, River Hills employs caregivers and handles the administrative details. This means that, as an individual, you can receive help without having to manage employment logistics.

How to Start
Receiving Services

Assessment

Complete a free MnCHOICES Assessment with your county or tribal authority, or a Managed Care Organization (MCO). This determines which waiver service(s) you qualify for.

Support Plan Creation

If you qualify for services, a case manager will work with you to create a Support Plan. This document outlines your unique goals, needs, and choices for care.

Service Authorization

Your case manager sends a Service Authorization to River Hills so that we can check your eligibility and ensure services will be reimbursed before our services can begin.
