What does River Hills Home Health do?
River Hills Home Health is a home health care agency based in Rochester, Minnesota. We provide personalized, hands-on support with daily living so individuals can remain safe, comfortable, and independent at home. As a state-licensed provider of home and community-based services, we work closely with individuals, families, and case managers to ensure care is reliable, consistent, and easy to manage.
Who do you serve?
We serve people of all ages who need assistance with daily living, including older adults, individuals living with disabilities, and families looking for dependable care for a loved one. Our team is here to help you understand your options and determine the best next steps.
Do you serve all of Minnesota?
We currently provide services in Minneapolis, Rochester, and surrounding communities throughout Olmsted and Hennepin Counties. If you're located outside these areas, please contact us. We'll be happy to discuss your situation and point you in the right direction.
How do I know if I qualify for services?
Most people who come to us qualify if they meet a few basic criteria:
- They have been assessed as needing support with everyday activities.
- They live at home or in a community setting, not a care facility.
- They are enrolled in Medical Assistance or a qualifying Minnesota waiver program.

Do I need a case manager or assessment?
Yes, both are part of the process. Before services can begin, you'll need to complete a needs assessment through your county or managed care organization. After your assessment, you'll receive a Notice of Action letter in the mail outlining the services you're eligible for. A case manager is also typically involved in coordinating your care plan. If you don't already have one, we're happy to help you get started.
What if I don't know what program I'm approved for?
Don't worry—you don't need to have it all figured out before reaching out to us. If you're unsure what you've been approved for, your case manager or county office can walk you through it. We're also happy to answer questions and help you better understand your options.

Do you help with caregiver scheduling and coordination?
Yes. We handle all of the scheduling and coordination behind the scenes so you can focus on what matters. From matching you with the right caregiver to managing any changes along the way, we take care of it all.
Can services change if needs change?
Yes, absolutely. We understand that needs can change over time, and we can adjust your care plan to reflect that. If your situation changes, you can request a reassessment through your county or managed care organization to update your services. We'll work with you and your case manager to make sure the transition is as smooth as possible.
